    Reporting an incident

    If you want to report an incident in the park, please do so using the form below.

    Reporting incidents is helpful as it allows the park management team to capture important management information and, where possible, take appropriate action.

    Incident report forms must be completed by first-hand witnesses.

      Report an incident

      Use this form to report:

      • Near misses.

      • Site accidents.

      • Serious breaches of site conduct rules e.g. bad driving.

      Note, accidents involving personal injury may need to be documented on a different form, available from the Bute Park Education Centre (CIS). Seek advice from Park Manager

      Where, what, who etc. Be as detailed and specific as you can.

      Importantly you should provide an identifying description of the people/vehicles involved, e.g. number plates, make, model and colour.

      If weather or local site conditions were relevant, include those too.
